Radiology faces a critical isolation crisis: 70% of radiologists report burnout (RSNA surveys), driven by solo reads, limited peer access, and inefficient case discussion workflows. Existing tools — grand rounds, email threads, static forums — are slow, formal, and provide no incentive for meaningful participation. The client needed a mobile-first platform that could instantly verify professional credentials via the CMS NPI Registry, fully anonymize patient data at upload, facilitate real-time encrypted one-to-one and group collaboration across subspecialties, and incentivize high-quality contributions through a transparent blockchain rewards system — all while maintaining strict HIPAA, CCPA, CIPA, CMIA, and NY SHIELD Act compliance.
We architected a React Native cross-platform application targeting iOS, Android, and macOS (Apple Silicon), backed by Supabase for real-time database, authentication, file storage, and row-level security (RLS). The verification module integrates directly with the CMS NPI Registry for automated credential validation of attending radiologists, plus program confirmation for residents and fellows. We implemented end-to-end encrypted messaging supporting both one-on-one direct messages and WhatsApp-style group chats for residency cohorts, fellowship groups, teleradiology teams, and private study partners — all maintaining full user anonymity. The Radcoin (RAD) rewards system was built on a custom ERC-20-inspired smart contract deployed to the Polygon network for low gas fees and high throughput, with dynamic conversion rates linking quality ratings (1-10 scale) to token distribution. A comprehensive dashboard tracks Radcoin balance, comment ratings, badges, and professional milestones. The UI supports four themes (Light, Dark, Blue, Green) optimized for clinical reading environments, smart notifications keep users informed without overwhelming workflow, and all uploaded cases auto-purge after discussion with full audit logging throughout.

The platform is built on a three-tier architecture spanning mobile, backend, and blockchain layers. The React Native mobile layer delivers a native-feeling experience across iOS, Android, and macOS with four theme variants and real-time collaboration features. The Supabase backend provides PostgreSQL with row-level security (RLS), real-time subscriptions for chat and notifications, secure file storage for anonymized radiology images, and built-in authentication integrated with the custom NPI verification pipeline. The blockchain layer consists of a custom ERC-20-inspired smart contract deployed on Polygon for low-gas, high-throughput RAD token transactions, with an in-app non-custodial wallet and dynamic conversion rates that adjust based on community growth. End-to-end encryption protects all messaging channels, TLS 1.3 secures data in transit, and database-level RLS policies ensure data isolation between users — all backed by comprehensive audit logging for regulatory compliance.
The project launched in phases starting Q4 2025 with the MVP including NPI verification, case posting, comment-based RAD earning, and one-to-one messaging. The Growth phase (Q1-Q2 2026) added group chats, four UI themes, and expanded the verified user base. The Expansion phase (Q3-Q4 2026) introduces sponsored cases and group chat v2. The Maturity roadmap (2027+) targets CME pathway integration and potential RAD transferability subject to governance and regulatory approval.
